The 2024 International Liaison Committee on Resuscitation (ILCOR) update of the Out-of-Hospital Cardiac Arrest (OHCA) Utstein template introduces critical clarifications and enhancements to existing definitions, reflecting recent advancements in emergency response practices. This update aims to provide valuable reporting tools to enhance the quality and consistency of OHCA data.

Key updates include:

  1. An Emphasis on Dispatchers and System Enhancements: Highlighting the vital role of dispatchers in recognizing OHCA and providing CPR instructions, the update incorporates changes to systems and practices, including community volunteer programs and high-performance CPR.
  2. Expanded Data Collection: Patient core elements have been broadened, offering additional data options. The supplemental list now includes details on who first provided CPR and defibrillation, capturing crucial early intervention data.
  3. Detailed Process Variables: Process variables are now divided into intra-arrest and post-resuscitation phases, with expanded time points to accurately estimate the duration of arrest. This is important for statistical adjustments in observational research examining the impact of various interventions.
  4. Updated Post resuscitation Care: Definitions and coding of post resuscitation care elements have been updated to align with the latest treatment recommendations and the 2019 in-hospital cardiac arrest Utstein template.
  5. Aligned Outcome Measures: Outcome items have been modified to align with the ILCOR Core Outcomes Set for Cardiac Arrest (COSCA) publications, ensuring standardized outcome reporting.
  6. Practical Reporting Tools: Usable flowcharts are provided for assessing system effectiveness (attempted resuscitation) and system efficacy (Utstein comparator group).
  7. Minimum Dataset for Low-Resource Settings: A simplified dataset is available for regions with developing emergency systems, promoting standardized reporting in varied healthcare environments.
  8. Research Reporting Guidance: A Consort-style checklist is included to guide comprehensive reporting in research publications.

This update underscores our commitment to improving OHCA outcomes through enhanced data quality, comparability across studies, and informed emergency response practices.
